Before the verification
You should receive the completed facility assessment with applicable uploaded documentation no later than 10 working days prior to the scheduled onsite verification date/window.
 
Onsite verification
An onsite verification never takes more than 1 working week. You can find an overview of required person-days (related to facility size) in the Verification Protocol. If more than 4 person-days are required (due to the facility size/operations), the VB needs to assign more than 1 Verifier to the verification. You will schedule a visit to the facility to complete the verification. This scheduling happens offline.
 
After the verification
In the closing meeting you discuss the findings with the facility and resolve any issues before submitting the verified data on the Accredited Host platform. You have 10 working days after the onsite verification to submit the verification findings on the Accredited Host (AH) platform. You can choose to use the offline Excel Tool to write the verified assessment report. However, a verified assessment report can only be completed online. The AH runs checks to ensure proper completion of the report when it is submitted online. Make sure you reserve some time to correct any omissions/mistakes when writing the report offline and uploading the Excel on the AH platform.
 
After you submit the findings from the verification, the facility has 14 calendar days to approve or dispute the verified assessment report. Ideally, they will inform you within 48 hours of any question level issues that need to be resolved. This can happen offline. Note that you can still make changes to the verification data during those 14 days. Any changes you make to the report after completion/ during the 14-calendar day facility review phase must be agreed upon by the facility. The facility must be informed about the changes and when they took place, so the facility can go back to the review (with the implemented changes) and accept the verification before the 14-calendar review period is over. If the facility does not take action within the 14-day review and approval period, the Accredited Host will automatically accept the verified assessment report. Note: once finalized, the report can no longer be changed.
