No, you need to submit the findings of the verification online. We do provide the possibility of using an offline tool to collect the findings.

Access the Tool
You must first access the self/ joint-assessment completed by the facility by going to their assigned Accredited Host (AH) platform. Once you start the verification on the platform, you have access to the self/ joint-assessment document online as well as offline through download from the AH site.

Completing the Tool
You can complete the verification online and, if necessary, download to the offline version, which will keep any answers that were already entered and saved online. You can also complete the self/ joint-assessment offline and upload to the online platform, which will keep any answers that were already entered and saved offline. This process of going from online to offline to online can be repeated as many times as necessary.

We recommend filling the verification in online through the Accredited Host platform as quality checks are in place (and it is more efficient), but you can make use of the offline Tool by downloading the Excel file on the Accredited Host platform. Note that due to the quality checks on the AH platform, you may need to revise the data you filled in offline after uploading it.

Submitting the Tool
The final step is always to finish the verification online, as the AH runs checks on SLCP verification completion requirements. The checks may require you to review and edit answers. At this point, you must also upload applicable documents/ photos to the AH platform.
